Transition Care is a short-term aged care program designed to support recovery after a hospital stay. It helps older people rebuild strength, regain confidence and improve functional independence, potentially reducing the need for longer-term home care services.

Transition Care may be delivered at home or in another suitable setting, depending on individual needs and local service arrangements. The Department of Health and Aged Care advises that Transition Care can be provided for up to 12 weeks.
