Support at Home is a major Australian Government aged care program introduced in November 2025 to help older people remain independent, safe and connected while living in their own home. The program provides access to flexible home care services, equipment and home modifications based on individual needs and assessment.

For many families, the key benefit of Support at Home is the ability to create a coordinated aged care at home plan – combining clinical care, everyday support and safety improvements into one streamlined approach.

Services may include nursing care, allied health, personal care, domestic assistance, transport, meals and assistive technology, depending on eligibility.

All Aged Care supports you by explaining how Support at Home works, coordinating services and adapting care as needs change.

Important note for clients currently on CHSP: the Government has advised CHSP will transition into Support at Home no earlier than 1 July 2027. If you’re unsure what this means for you, we can help explain what to expect and how to plan ahead.
